As a parent/guardian of a new or transfer student, we hope your transition to our program and school is a smooth and positive experience. With your help we plan to maintain a safe and healthy environment for your child to learn and grow.
The Grand Rapids Oral Deaf Program is located at Shawnee Park Elementary School and is part of the Grand Rapids Public School System. Although a GRPS school, we operate through Kent Intermediate School District, and as such, we invite you to contact us if you're interested in enrollment. Please call (616) 819-3070 and we'll be happy to answer your questions and start the process.
In order to enroll your child(ren), we will need the following:
1. Proof of residency in the Kent Intermediate School District
(e.g., buy/sell agreement, rental agreement, utility bill)
2. Original birth certificate
3. If divorced, copy of custody agreement
4. Complete immunization records (see below)
5. Copy of most recent report card and/or high school transcript, if applicable
It is very important that the emergency phone numbers and contacts be current in the event that we cannot reach you. In addition to accident or injury, it is important that you have a designated person or network available for your child in cases of early school closings or inclement weather.
If your child has a medical condition i.e., diabetes, epilepsy, asthma, acute allergies (food, insect bites, animals), or physical disability, please inform the nurse, teacher, or school secretary to insure a safe, healthy school environment.
You may submit a Health Appraisal form provided by your health care provider documenting vision/hearing screening. Your provider will also provide you with the "Official" immunization record. This must be completed by the first day of school.
There have been recent changes in immunization requirements for Young 5's and kindergarten students and children who are 11 years old. Please check with your health care provider.
